Voucher Application
What is the new blood program all about?
It is a program that will help our families who receive blood products to offset the expenses they incur.
How does the program work?
CUMC members who are able to give blood go to any Lifeblood collection site and give blood in the church’s name. For every 25 donations (whole blood or platelets) or 12 donations of double red cells donated in CUMC’s name, a voucher for up to $100 credit will be set aside for the use of a church member.
So if someone in my family has a blood transfusion or receives units of blood or platelets—something like that—could I receive some help with the hospital bill?
Yes. You would need to fill out the
application and submit the portion of your hospital bill that indicates a blood product was given, as well as your insurance coverage. These documents will have strict confidentiality observed but are necessary for the review team to determine the amount of vouchers that will be given.
Is this for members only or can anyone participate?
This program is only for church members—either through profession of faith, baptism, or transfer of membership. Even if you are unable to give blood, you can be a recipient of a CUMC voucher to assist with your costs.
What if my family wishes to participate under our own name?
When you donate, still say you wish credits to go toward CUMC. According to the family programs, when you give two donations, you receive up to $100 voucher for your family. However, those same two donations will go toward the 25 needed by the church for a voucher.
Sometimes I am asked to donate blood in someone's name. Will the church and my family still be given credit for the donation?
Yes. You need to identify your family's account, ask them to add your donation to the church's tally and then tell them the name of the person in whose honor you are giving. They will send that person a note to inform him/her of your donation.
How do I know if I’m qualified to give blood?
Go to
www.lifeblood.org or call 1-888-lifeblood to learn the criteria to be a donor.
When will the program start?
We will be able to review our first applicants at the beginning of December.
